A red X under the “PHS Shared” column indicates that the department does not require a PHS through PST, so we will not share your PHS with them. If you are not applying to any agencies that require the PST PHS, you do not need to complete this form in your account. However, the department(s) may ask you to fill out a similar form as part of their own hiring process.
