# What if I don’t know all the contact information for references/past jobs/roommates?

For your PHS, please make your best effort to provide all the requested information. If you are unable to obtain certain details, include a note at the bottom of the section explaining why the information is missing. You may also be asked by your investigator to explain any missing information during your interview.
