# eSOPH Questionnaire Tips

## Follow these guidelines to complete your eSOPH questionnaires accurately and avoid delays in processing.

#### **References and Employment**

- If you are unable to provide a required reference’s contact information (phone number or email address), use the Special Instructions box to explain why.
- Provide as much context as possible so your investigator understands the situation. 

#### **Neighbors**

- If you do not have a current neighbor reference, please explain the reason in the Special Instructions box.
- Include any relevant details, such as recent moves or living arrangements that make this information unavailable. 

#### **Residences**

- If any of your relatives also lived with you, you do not need to list them again as housemates if their contact information was already entered in the Relatives questionnaire.
- Instead, use the Special Instructions box to note which relatives lived with you at each residence. 

**Before submitting your questionnaire:**

- Review all sections for accuracy and completeness.
- Use the Save button frequently to prevent data loss.
