Completing the Personal History Questionnaire (PHQ)

Instructions for completing your Personal History Questionnaire (PHQ) are provided in the eSOPH email invitation you received from PST at the beginning of the process.

If you attempt to log in and receive a message stating that you’ve already completed the questionnaire, please reach out to your assigned PST Investigations Representative for assistance.

The PHQ must be completed within three hours of starting. Once you begin, the timer will start, and you will not be able to exit or change your answers. Please ensure you have an uninterrupted block of time before you begin.
